Quality is achieved when:
A. the requirements have been met.
B. customer expectations have been exceeded.
C. the customer indicates acceptance of the product or service.
D. the customer stops asking for additional features.

No doubts, A.
B: Customer expectations are important only before the Scope is specified. When it's specified and baselined customer expectations are important only for customer, not for the project manager.
C: Customer may indicate un-acceptance of the product and may refuse to pay for it, but if the requirements are met -it's a question of a debt collector, not a PM.
D: This is not about quality at all.
In general, Quality is a relation of the number of requirements met to the number of requirements specified.

My answer is C.
This doesn't means that quality will not be planned. Customer's expectations will be recorded and added to project scope. So when this expectations are exceeded and approved - Quality is achieved.
A - What requirements? Quality? Functional? Supplementary? This answer can't be right anyway.
B - This is not full.
D - This "additional" features are rather Scope than quality requirements.
